Follicular Tracking

Follicular tracking is a key fertility procedure used to monitor the development of egg-containing follicles in your ovaries, from their early (primordial) stages to maturity (dominant follicles). This process helps identify when your eggs are nearing maturity, aiding in the timing of ovulation and optimizing the timing for egg retrieval in IVF procedures.

Procedure Overview:

Follicular tracking is performed using a transvaginal ultrasound combined with blood tests to measure levels of estrogen and progesterone. The procedure is quick, typically lasting around 10 minutes.

What to Expect:

  • Preparation: Please arrive with an empty bladder. You will be asked to remove your underwear and either wear a gown or be covered with a sheet.
  • During the Scan: You will lie on a bed with your feet on a lower platform. A small, sterilized transvaginal transducer will be gently inserted into your vagina with your consent. The probe will be moved to visualize your ovaries and count the follicles. The sonographer may gently press on your lower abdomen to clear any bowel gas. For detailed information, refer to the transvaginal ultrasound information sheet.

Tracking for Timed Intercourse:

In a natural cycle, follicular tracking helps identify the best time for intercourse when a leading follicle is present (typically 22 mm in size, ranging from 17-27 mm). Medication may also be used to trigger the release of the mature egg.

Tracking for IVF:

During IVF treatments, your ovaries are stimulated with follicle-stimulating hormone (FSH) to produce multiple mature follicles. This is closely monitored with 2 to 4 ultrasound scans and regular blood tests. The timing of egg collection is determined based on these results and will be communicated to you by your IVF nurse or clinician. The egg retrieval procedure is typically performed under anesthesia using a transvaginal ultrasound-guided technique. In some cases, if vaginal access is challenging, the procedure may be done transabdominally.

Who Might Benefit from Ovulation Tracking?

  • Women with regular cycles who have been trying to conceive without success.
  • Women with irregular cycles seeking to determine the optimal time for conception.
